Home with Support
Match to equipment and essentials; link to family and voluntary help.

Step-down Housing
Reserve a short-stay bed (Discharge-to-Assess, crisis homes, and so on) to complete assessments safely.

Care Homes
Care Homes or Specialist Mental Health facilities

Domiciliary Care
Book home care agency visits (for example, three times daily for two weeks or for the agency to continue care, for example, people living with disabilities) before the patient leaves the hospital.

Equipments, fall detector suppliers, domiciliary agencies (mainly pathway 1 but other pathways are not omitted if needed)
